Subject: Important Notice Regarding Medical Requests and Medical Records

Due to the overwhelming volume of current medical requests, we are no longer able to accept any additional requests at this time. We are unable to provide timely and medically appropriate responses or follow-through, and we must prioritize patient safety as we transition toward closing the practice.
You may continue to access your patient portal to view office notes, laboratory results, medication lists, and other available records.
Please visit www.harperurology.com and click on “Patient Portal” for access.
All complete medical charts will be securely transferred to our designated Custodian of Records, Cariend (www.Cariend.com). Due to the large number of records involved, Cariend will receive the full charts within 6–8 weeks.
Once the transfer is complete, you will be able to request and obtain your full medical chart from Cariend at no cost.
We encourage you to check our website for updates as we progress through this transition.

Dear Valued Patient,

I want to share some important updates regarding my medical practice. After many wonderful years of caring for this community, I have made the decision to retire. The last day of patient care will be Friday, December 5, 2025.

For patients needing ongoing or new urologic care, I would like to offer a few recommendations of trusted urology providers in the area:

You may use your patient portal to request medication refills and to access office notes and lab results. Due to the overwhelming volume of medical records requests and our limited remaining staff, we will be adjusting our records fulfillment process as follows:

• At this time, we are able to provide your most recent office notes, laboratory results, and test/diagnostic reports upon request.

Before scheduling with a new urologist, please check with your insurance company to confirm which providers are in your network and covered under your plan. You are welcome to choose any provider you prefer, and we will gladly assist in transferring your medical records to their office when needed.
